What are the Effective Solutions for Significant Indicators of Aging?


The neck and décolleté area are one of the most significant indicators of aging. Cumulative photodamage, extrinsic factors and chronological aging accelerate the aging process in these areas. Ultraviolet rays are the most effective factor in skin aging; hormones, smoking, excessive alcohol consumption, dry skin and gravity also trigger aging. In chronological aging, the renewal rate of the epidermis decreases, the epidermis thins, the number and function of fibroblasts in the dermis decreases; thus, collagen and elastin synthesis decreases, the skin loosens and wrinkles form.

Botilinium Toxin Application
In the Neck Area:
The platysmal muscle is responsible for the transverse lines in the neck. With the application of botulinum toxin (BTX), the lines in both the horizontal and platysmal bands are reduced. 1-2 units of BTX are applied to the horizontal lines at 1-1.5 cm intervals to form an intradermal or subcutaneous papule. For platysmal bands, the muscle is highlighted by saying the letter ‘E’ or clenching the teeth and injections are made at 2 cm intervals. A total of 75-100 U of BTX can be used, overdose should be avoided. The ideal patient is someone with a thin neck structure and good skin elasticity.

In the Décolleté Area:
If there is fluctuation in the décolleté area when showing the lower teeth and grimacing, inferior platysmal muscle activity is evident. Injections are applied to 12-16 points at 1 cm intervals, the total dose varies between 75-120 U. BTX treatment is not recommended in cases where wrinkles are caused by gravity or sleeping habits. Hematoma and erythema may be observed among the side effects.

In BTX applications, as a result of overdose or deep application, sternocleidomastoid muscle fibers are affected and weakness and dysphagia may develop in the neck flexors, so dose control is important.

Mesotherapy
Cocktails containing hyaluronic acid, multivitamins, amino acids, minerals, coenzymes and nucleic acids are applied intra-epidermally with a 30/32 gauge needle or mesotherapy gun. It can be applied every 2 weeks for 2 months or every 6-8 weeks for 3 months; maintenance treatment is done every 2-6 months. Mesotherapy supports skin metabolism and circulation by increasing fibroblast activity, thus increasing skin moisture and elasticity, and improving skin tone.

Microfocus Ultrasound (MFU-V)
It is a non-invasive method that can be applied to all skin types. It stimulates collagen contraction and restructuring by creating controlled thermal damage at depths of 1.5 mm, 3.0 mm and 4.5 mm. It is approved by the FDA for tightening the neck and submental region. Its effectiveness has also been proven in décolleté wrinkles.

Radiofrequency (RF)
Heat energy is applied to a depth of 0.5-3 mm with monopolar, bipolar, tripolar and multipolar devices. Tissue regeneration is provided with collagen contraction and fibroblast stimulation. There is also a lipolysis effect due to the breakdown of triglycerides. Minimal side effects and short recovery time are the advantages.

Pulsed Electromagnetic Fields (PEMF)
